This temporary grant position is at Jefferson County Department of Social Services. Supervision and support is provided through JC DSS Permanent Housing programs. The case manager works closely with Permanent Housing program staff to assist high-risk individuals to obtain safe and secure housing. The caseload consists of homeless adults in the Permanent Housing programs. Case manager works with these individuals to maintain housing and prevent homelessness. Intensive assistance and coordination services are provided to connect these individuals to necessary services in the community. Services are individualized to meet the specific needs of each person in a culturally sensitive way. The case manager encourages and assists the individual to develop natural community supports and use community resources to encourage integration into the community. This position may require the use of personal vehicle to transport individuals when an agency vehicle is not available.
