Description:Job Description Summary:The Homeless System Navigator is responsible for coordinating navigation efforts across Jefferson County for individuals experiencing homelessness. This position will demonstrate and utilize knowledge of housing and stabilization outcome principles, practices, and techniques. Navigators will support individuals with accessing shelter, housing, mental health services, basic needs, treatment and other necessary services. Navigator will utilize Homeless Management Information System (HMIS) for data collection and reporting. Navigator will collaborate with community partners and provide coordinated street outreach and services to highly vulnerable populations.
Schedule:This position operates primarily in-person on a four- or five day work week, which can be discussed with the hiring manager. Offsite community outreach and mobile work is required. Some flexibility is necessary on an as-needed basis.
Compensation:Hiring Range: $25.00 - $27.00 USD HourlyCompensation will be determined based on education, experience, and skills.

Essential Duties:DATA MANAGEMENT: Manages and enters data in spreadsheets, including in the Homeless Management Information System (HMIS), and analyzes data using statistical software. Conduct assessments to gather qualitative and quantitative data regarding services for individualized planning. Maintain records within HMIS to accurately reflect progress towards housing stability. Works with Regional Homeless Coordinator to achieve data integrity. Supports expediting services for clients in need through research and present data collection. Research national evidence-based practices in the areas of housing strategies. Interprets complex data and information into distinct formats using tables, graphs, or other visual aids.CASE MANAGEMENT/ COMMUNITY OUTREACH: Facilitates, organizes, participates, and evaluates in the regional homeless navigator network. Facilitates goal development with clients by providing ideas and input for housing and stabilization outcome goals. Be an active participant in Heading Home and workgroups to advocate on behalf of clients to remove barriers, provide resources, and achieve stabilization goals. Attend trainings with state and local counties and community agencies servicing individuals. Provides navigation support as a wraparound facilitator and liaison with any other system or professional the customer is involved with. Assists with homeless referral and resources support, entry and exit follow-up plan development, as well as facilitation and participation in individualized service and support teams. Meet directly with individuals to provide resources and knowledge of service agencies to achieve outcome goals. Visit camps, parked cars, and other outreach sites to meet unhoused people where they are at. Responds to navigation inquiries from families, community, and professionals. In-person visits to office environments that assist with direct homeless service delivery. Maintains resources and eligibility requirements for services available to individuals, including community and public assistance programs. Coordinate with Sheriff's department and other community programs and outreach to unhoused community members. Demonstrate ability to establish and maintain rapport with landlords, or potential placements, for housing conversations with involved professionals. This includes phone calls and other electronic communication, referral and application support, and effective problem-solving. Assist in the regional coordination of the Point-In-Time Count effort for Jefferson County. Coordinate extreme weather and severe weather to align, streamline, and enhance efforts. Other duties and responsibilities as assigned.

Minimal Qualifications: A bachelors degree in related field A minimum of three years of experience; volunteer or lived experience Or an equivalent combination of education & experience.
Preferred Qualifications:- A true passion for serving and working with people
- Preferred experience working with individuals in poverty, Human Services system, Justice Services system, and Homeless Population. Experience communicating with varying levels of audiences (vulnerable population, community partners, etc).
- Demonstrate exceptional organizational skills and an ability to manage numerous projects simultaneously
- Demonstrate ability to maintain composure and professionalism during stressful situations
- Demonstrate ability to make decisions recognizing established procedures and practices, and to use resourcefulness and tact in addressing new situations and problems
- Demonstrate ability to establish effective working relationships with individuals and groups participating in the Homeless Navigator network, as well as associates, fellow employees, supervisors and clients and guests
- Experience with documentation and case management. Diffuse and/or de-escalate situations to reach a mutual resolution suitable to all concerned. Strong time management and prioritization skills.

- Demonstrate team spirit and a willingness to assist coworkers when necessary
