Job Summary:

Providing assistance that is not medical in nature, but is based upon assisting the client in

meeting the demands of living independently and maintaining a personal residence, such as

companionship, cleaning, laundry, shopping, meal preparation, dressing, and bathing.

A Companion or Homemaker shall not provide personal care/hands-on care.

Duties and responsibilities:

Observation of client functioning and reporting changes to their supervisor or employer or to a

person designated by the client.

Providing assistance with household chores (light housekeeping), including cooking and meal

preparation, cleaning and laundry.

Providing assistance in completing activities such as shopping and appointments outside of the

home.

Providing companionship.

Completion of appropriate records documenting service provision.

Reminding the client to take medication.

And other non-medical, non-hands on tasks as directed by the Manager or Supervisor.

Minimum/ Acceptable Requirements:

• Must be willing to undertake a minimum of ten hours of training. Five hours of training

shall be provided prior to the first assignment, and the remaining five hours shall be

provided within the worker's first 30 days after the start of employment.

• Revival Home Care Agency LLC will accept proof that the worker has successfully

completed a training program at or through another licensed home services agency

1 of 3within the prior year (previous 365 days) instead of providing or arranging for training,

including a CNA who is approved on the Health Care Worker Registry.

• Must successfully complete a competency test that covers the topics included in the first

5 hours of training before being given the first assignment.

• Must successfully complete a competency test following the remaining 5 hours of

training.

• Must not have a disqualifying background check under the requirements of the Health

Care Worker Background Check Act without a waiver;

• Must produce a copy of their Social Security card; and

• Must have a visa or proof of citizenship in compliance with federal requirements for

employment.

Additional requirements:

• Must be able to stand at ease for a minimum of 20-minute intervals.

• Must be able to climb one flight of stairs comfortably while carrying at least 15 pounds.

• Must be able to walk and run-on rough ground and uneven surfaces.

• Must be able to lift at least 30 pounds from the ground to a chair or bed.

• Must be able to pull and push at least 30 pounds.

• Must be able to bend, kneel, squat, twist, squat, crawl, and climb.

• Should have a compassionate and caring attitude.

• Must be self-motivated and eager to learn.

• Should be reliable and an excellent timekeeper.

• He or she must also possess excellent written and verbal communication skills.

• Must have the ability work under little or no supervision.

• Must have the ability to work well under pressure and change.

• Should be non-judgmental and willing to respect clients and co-workers as well.

• Must have a full understanding of Private Duty Non-Medical legislation and regulations.

• Computer skills are also a necessity.

• Must have a clean driving history and must possess a current driver's license.

• Must have a dependable mode of transportation/vehicle.

• Negative TB test or chest x-ray upon hire with written documentation of “history of

positive PPD by a physician”, provide chest X-ray per CDC guidelines and annual TB

symptom check off by a physician.
