Job Type: RegularTime Type: Full timeWork Shift: Day (United States of America)FLSA Status: ExemptWhen you join the growing BILH team, you're not just taking a job, you're making a difference in people's lives.Job Description:The Hospice Admission Nurse plans and delivers appropriate palliative & hospice care to patients through assessment, intervention, implementation, and evaluation while effectively interacting with patients, family members/significant others, and interdisciplinary team members to maintain standards of professional nursing and clinical competency.Essential Duties & Responsibilities:• Determine the appropriateness of referrals, coordinate and conducthospice pre-admission activities;• Describe and explain hospice services to a potential patient/family andelicit response(s) to questions regarding patient/family attitude(s) towardhospice care, physical and psycho/social/emotional status in order to putthe patient and family at ease about choosing hospice and fully informthem about the program;• Gather pertinent information necessary for admission;• Speak to patient/family about insurance coverage and financial obligations,using knowledge of hospice fees, cost and reimbursement sources;• Promote hospice services to ensure appropriate referrals to hospiceprogram to discharge planners, physicians, other health care providers andreferral sources;• Manage care of patients, coordinate all services and facilitatecommunication between all providers.• Provide skilled nursing care and prescribed treatments to patients andfamilies in their home.• Maintain productivity expectations.• Provide documentation of services utilizing problem-oriented record.• Plan for continuity of care with nursing team members, patient, family, andother disciplines.• Use opportunities for teaching nursing care, comfort measures and healthpractices to the patient, families and others.• Instruct and supervise care given by the Home Health Aides.• Communicate and coordinate with other health and social agencypersonnel to assist in planning, implementing, evaluation service to thepatient and family;• Complete Hospice Item Set (HIS) time points. Accept accountabilityfor patient satisfaction scores and quality of care outcome measureresults;• Initiate the plan of care, as assigned, and make necessary revisions aspatient status and needs change;• Administer medications and treatments as prescribed by the physician;• Counsel the patient and family in meeting patient needs related to physical,emotional, and spiritual needs;• Provide information to the patient and family regarding the dying processand expectations.What You'll Need:• BSN strongly preferred.• Current valid license to practice as a registered nurse in Massachusetts.• Current Basic Life Support Certification• Valid Massachusetts driver's license• 1+ year of recent professional nursing experience• Hospice or end of life care experience preferred.What You'll Get:• A highly competitive salary & benefits package, including generous PTO,403(b), and tuition reimbursement;• A reasonable geographic territory with strong clinical support resources;• A highly inclusive, diverse team that values the input of all staff to provideexcellent patient care.As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
