James River Hospice is actively seeking a compassionate and highly committed Hospice Admissions RN to join our growing team! You will be responsible for evaluating patients for hospice care eligibility, ensuring they meet the criteria, and providing comprehensive explanations of hospice services to patients and their families. You will play a crucial role in coordinating the initial care plan and facilitating a smooth transition into hospice services. (PTO policy rewards longevity with increases each year)Competitively priced health plan, vision and dental, partially employer paid!Company paid short and long-term disability and life insurance (up to $50,000 in coverage)401K with employer matchCareer advancement opportunitiesProactive Health Management Wellness Program (PHMP)We Pay for CPR Recertification!Brief Overview of Responsibilities for Registered Nurses:As an admissions nurse, you will introduce and enroll patients and families into hospice services, assess their needs, and plan/co-ordinate care to address end-of-life needs in their homesProvide education to patients and families on treatment plans and discharge instructionsFacilitate a smooth transition to hospice care by educating and orienting patients and families, providing insights into the disease process, and preparing them for end-of-life home care.Evaluate available medical records and perform an initial assessment to determine hospice eligibility according to Medicare guidelinesCreate a care plan based on the end-of-life goals of the patient and family, utilizing nursing diagnoses and incorporating palliative nursing actions.Other duties as assignedQualifications of Registered Nurses:Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of VirginiaAt least one year of hospice experience is required.Graduate of an accredited nursing school.Must have a valid driver's license and a reliable car available for use during work hours.If you are interested and are a caring, compassionate nurse who can make a difference in the lives of our patients, then we would be interested in meeting you!!
