Hours: 40hrs
Shift: 10a-6:30, 11a-7:30 or 12-8:30pm with weekend and holiday rotation; Every 4th weekend, 2 holidays per year
Location: Southcoast Business Center - Fairhaven, MA

- $8,000 New Hire Sign On with 1+ years of Hospital RN Experience OR Less than 1 year Hospice/Palliative experience
- Rehires termed greater than 1 year are eligible

ResponsibilitiesUnder the supervision of the Director, Supportive Care Services or designee, and within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and families. Assess, develop, implement and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physician*s orders, Plan of Care and Hospice and Home Care policies and procedures.Qualifications
- Graduate of an NLN accredited school of Nursing is required.
- Bachelor of Science in Nursing is preferred.
- Two years recent hospital, medical/surgical or other relevant nursing experience required.
- Hospice & Palliative Care Certification is preferred.
- Demonstrated knowledge of I.V. pump, and central line maintenance preferred.
- Has knowledge and skills necessary to provide care, which is appropriate to the population of patients served.
- Proficient computer skills, including experience with various software, database and electronic records programs is preferred.
- Current RN licensure in the Commonwealth of MA and RI is required.
- Hospice & Palliative Care Certification will be required within eligibility timeframe of 2 years if not certified at time of hire.
- Basic life support (BLS) certification is required.
- Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired
Pay RangeUSD $36.90 - USD $52.64 /Hr.Rotation Schedule Requirementevery 4th weekend, 2 holidays per year
