Hours: Monday - Thursday 5pm - 8am and 5 pm Friday thru 8 am Monday, every other weekResponsible for the care of hospice patients during non-business hours including, but not limited to: triaging patient phone calls, providing skilled nursing interventions and supportive care in the patient's place of residents for evening and night after hours duty in accordance with the patient's respective plan of care and based on need. Provide support and follow-through to the patient's family during any actively dying and/or death visit for the hospice team. This position is full-time, benefit eligible, and will be based out of our Detroit Lakes office and will serve the surrounding communities.
