Jacksonville, FL 32257Under the supervision of the Nursing Support Services Manager and the direction of the Primary Nurse, the Hospice Aide Float provides direct patient care in the home during crisis or transition period in providing end-of-life care/support for patients and families alike.All employees assigned this job title must be willing and able to flex their work location within and between the any Community Hospice of Northeast Florida, Inc. (Community Hospice) location or team at any time if there are no cases on assigned days of work.
Primary Responsibilities:- Provides personal care including light housekeeping, and meal preparation as directed in the Hospice Aide care plan.
- Demonstrates knowledge of standard precautions and ability to perform basic infection control procedures.
- Demonstrates to family/caregiver proper techniques to meet the personal care needs of the patient.
- Demonstrates awareness and maintenance of Universal Precautions.
- Provides emotional support to patient/caregiver.
- Teaches patient/caregiver as appropriate to situation, plan of care.
- Demonstrates knowledge and appropriate use of medications, including medication administration.
- Demonstrates competency in skills identified, maintaining completion of annual competency of these skills per agency standard.
- Notifies supervisor immediately if Crisis Intervention care case ends.
- Reports changes in patient's condition or care needs that require follow-up to PCC. Can include physical, spiritual, social and emotional status.
- Maintains patient confidentiality at all times.
- Other duties as assigned.
Required Licenses/Certifications: - Current licensure as a CNA in the state of Florida.
- Current CPR Certification.
- Must have AIDS 104 Certification.
- Current State of Florida Driver's license with driving record acceptable to Community Hospice's insurance carrier.
Education/Experience:Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
- High school diploma or GED equivalent preferred.
- Must have six (6) months experience of either home care, hospital, and/or long term care.
- Experience in caring for the terminally ill preferred.
- Must have reliable transportation.
