SUMMARYUnder the supervision of the Assigned Management, this position provides the quality care necessary to restore or maintain the patient's physical and emotional well being. The position assists with the physical and personal care of clients and their activities of daily living, including assistance with meals, maintenance of safety and environmental upkeep. Hospice Aides will assists in maintaining a supportive and empathetic environment for patients and families.HILDEBRANDT HOSPICE CENTERHildebrandt Hospice Care Center is an 11-bed facility at 2652 Ridgeway Avenue that embodies the promise of comfort and dignity by providing end of life care. Nurses, Evaluators and Hospice Aides work directly with patients and support their families through the loss of a loved one.   • High School Diploma or equivalent.
  • At least one year of Nursing Assistant or Home Health Aide experience.
  • HHA Certified or ability to meet one of the following eligibility requirements for Competence: Currently certified CNA, documented home health aide or nurse aide training and competency evaluation from an out-of-state training program, or home health aide with documented home health aide training and competency evaluation who has not been employed as a home health aide for 24 consecutive months
RESPONSIBILITIES
  • Patient Care & Service. Follows Hospice Plan of Care for each individual patient as directed by the RN. Observes patients' conditions and reports any and all changes in patient condition to the RN. Turns and repositions bedridden patients alone or with assistance. Feeds patients who are unable to feed themselves. Provides patients assistance with walking, exercising and moving in and out of bed, if needed. Provides assistance with personal care - bathing and grooming.
  • Planning & Communication. Offers support and comfort to families at the time of patient's death. Consistently demonstrates high standards of integrity by supporting the Lifetime Healthcare Companies' mission and values and adhering to the Corporate Code of Conduct.
EDUCATION:LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. PAY RANGE:$17.60 - $19.50CITY:RochesterPOSTAL CODE:14623The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
