Role Overview
The hospice aide is responsible for providing personal care under the direction of a registered nurse.

Responsibilities
- Document care provided according to organization policy
- Report any deviations from the Plan of Care to the Case Manager
- Use Durable Medical Equipment safely
- Take vital signs when on Hospice Aide Plan of Care
- Foster cooperative effort among personnel
- Provide proper care and observation of patient's skin
- Assist with ambulation, transfer, and range of motion
- Provide respite for patients family/caregiver when on-site
- Participate in QAPI activities
- Participate in IDG meeting
Qualifications
- Meet training requirements by the state
- One year experience as a CNA, hospice aide or home health aide
- At least 18 years old
- Must be a licensed driver with an insured automobile
- The ability to communicate well, both verbally and in writing
- Ability to work as a team and also independently
