DescriptionWhat We Are Looking ForJob Title: Hospice Aide Specialist - (Residence/Community) Schedule: Residence: 3 shifts per week / 12 hours each day, 7:00 am - 7:30 pm. Community: 5 shifts per week 8:30a - 4:30p Essential Job Functions/Responsibilities: Follows Home Health Aide plan of care as instructed by primary RN. Establishes priorities and organizes work and time appropriately. The Hospice Aide is responsible for observing patients and reporting/documenting these observations of the patient's status and the care or service furnished. Responds to patient/family/caregiver needs promptly. If unable to perform a certain task, reports to the primary care nurse immediately.Meets safety needs of patient by using equipment safely and properly, i.e. (O2, side rails, etc.). Maintains a clean, safe, and healthy environment. Performs hands-on personal care. Follows basic infection control procedures. Performs simple procedures as an extension of therapy or nursing services. Offers adequate nutrition and fluid intake. Assists in administered medications that are ordinarily self-administered. Takes pulse and respiratory rate as requested by RN Case Manager. Recognizes emergencies and initiates necessary emergency procedures. Turns and positions patient, providing proper care and observation of the skin to prevent breakdown. Reports basic elements of body function and changes in body function to the patient's assigned registered nurse. Reports changes in the patient's condition to a registered nurse or other appropriate skilled professional. Assists in ambulation and exercises as instructed by the primary care nurse. Offers and assists with bedpans and urinals and changing of adult protective undergarments. Adheres to the organization's documentation and care procedures and standards of personal and professional conduct. Provides companionship- holding conversations, playing games, reading out loud, etc. The physical, emotional, and developmental needs of and ways to work with the populations served by the hospice aide specialist, including the need to respect the patient and their privacy and property. Provides respite for patient/family/caregiver when on-site as appropriate. Acts as a member of the hospice interdisciplinary group. When caring for a patient residing in a Skilled Nursing facility, demonstrates understanding of facility policy and procedure as it relates to Hospice care and facility equipment Documents via Electronic Medical Record daily. Uses TigerText and email for staff communication. Any other task that the hospice aide specialist may choose to have the hospice aide specialist perform as permitted under state law.The above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job related tasks other than those stated in this description.RequirementsPosition Qualifications One year of experience in a hospital or nursing home setting. (preferred) Successful completion of recognized HHA Certification program required At least eighteen years of age and is a high school graduate or equivalent. Ability to read and follow written instructions and document care given. Displays initiative and able to work with minimal direct supervision. Has empathy for the needs of the ill, frail, and impaired. Understands philosophy of hospice care and needs of the terminally ill. Demonstrates tact, patience, and good personal hygiene. Licensed driver with automobile that is insured in accordance with the organization requirements and is in good working order. Possesses good interpersonal skills. Ability to remain calm during stressful situations. Possesses excellent organizational skills. JOB LIMITATIONS: Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required skills.
