Req Number 142896Job DescriptionPerforms various aspects of hospital care within assigned patient care unit under direct supervision of a Registered Nurse.Job ResponsibilityProvides direct patient care as defined in the New York Nurse Practice Act in the Home, Nursing Facility or Inpatient or Residential Setting.Applies hospice and nursing practice standards to patient care activities.Implements plan of care initiated by the Hospice RN.Provides accurate and timely documentation of patient services reflective of plan of care.Reports changes in patient condition or needs to clinical staff.Assess and provides patient and family/caregiver education and information.Participates in coordination of hospice services, reporting needs for hospice interdisciplinary group members, including but not limited to: Hospice Aide, OT, PT, Speech, Nutrition, Skin, Bereavement Coordinator, Volunteer Coordinator or Clinical Supervisor referrals/assessments.Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.Participates in organization's Performance Improvement Program.Assists with the admission, discharge and transfer of patients.Assists in post mortem care.Attends IDG meetings and other staff meetings and in-services, as requested.
