Job Overview: Day to day, you would be responsible for providing nursing care to assigned patients in health facilities, hospitals or other healthcare settings. Our nursing staff demonstrates their knowledge, critical thinking, and judgment. Services provided by the RN Case Manager include; being responsible for the clinical supervision of Aides, Licensed Practical Nurses, and student nurses in the providing care to assigned patients. The RN Case Manager works closely with patients at assigned facility and their families, RNs who are overseeing care of patients, other facility personnel, Intake & Referral RNs, Social Workers, LPNs, Aides (HCAs), interdisciplinary group members, physicians, physicians' office staff, community agencies.Required: Certifications/Licenses/Experience:
- Active Registered Nurse (RN) in Indiana or a compact licensed state
Preferred: Certifications/Licenses/Experience:
- Six (6) months of nursing experience
- Six (6) months of home care experience
- Case Management experience
