The Hospice Clinical Manager is responsible for assisting in the overall coordination and supervision of the plan of care for Hospice patients and caregivers, including home, nursing home, assisted living and inpatient care. The Hospice Clinical Manager coordinates with the Hospice IDG team, patient, caregivers, Hospice Medical Director, Primary Care Physician, Specialist or other providers to work towards plan of care goals. The Hospice Clinical Manager assumes responsibility for coordination of care, serving as a liaison between the Hospice IDG team, patients, caregivers, pharmacies, facility staff, physicians and other providers. The Hospice Clinical Manager oversees and monitors the continuum of care for all patient care based on provider orders, compliance, departmental policies and procedures, state and federal laws and regulations, accrediting agency best practice. The Hospice Clinical Manager actively attends, participates and leads meetings as appropriate. The Hospice Clinical Manager performs supervisory visits.
