QUALIFICATIONS
The Hospice Community Liaison is expected to establish and maintain a professional relationship with physicians, case managers, discharge planners, social workers, other health care decision makers, appropriate special interest groups, senior organizations, nursing home professionals, and other community–based referral sources by providing on-going education regarding hospice benefits, services, and care. The HCL meets organizational goals through the management of assigned book of business, professional relationships and knowledge of hospice benefits and services. Must possess excellent communication skills, both written and verbal, in addition, successful candidates should have the following:
q Hospice/Long term care experience preferred.
q Documentation of prior sales/marketing preferred.
q Ability to create own leads.
q Self-starter and ability to work with limited supervision.
q Time management skills and ability to meet deadlines.
q Knowledge and or experience in creating marketing presentations.
q Conducting public speaking presentations.
RESPONSIBILITIES
The Hospice Community Liaison will be responsible for generating program growth from their assigned book of business and agency revenue and census goals. In addition, the HCL will develop and implement programs to raise and establish hospice awareness by conducting personalized visitations to all primary referral sources and interested parties, and by representing the agency in community events.
A. Duties
The Hospice Community Liaison works in conjunction with the Manager of Business Development:
q To develop new business by assertively identifying, pursuing, and closing opportunities for formal or contractual relationships with physicians, organizations, and payors.
q Works with office staff to facilitate new business successfully up to the start of patient care.
q Implements book of business sales plan and coordinates with operational and clinical leadership to ensure extraordinary customer service and fulfill Willowbrook Hospice organizational mission.
q Coordinate with hospice staff and follow through with referrals to the hospice program.
q Provides presentations on the Medicare Hospice Benefit to potential referral sources as well as patients/families.
q Identifies key referral sources, including appropriate physician offices, and provides education on the services of the hospice program for terminally ill patients.
q Develops opportunities and conducts in-service presentations to hospital and other appropriate referral sources regarding hospice services, admission criteria and how to make a hospice referral.
q Describes and explains hospice services to potential patients and families and elicits responses to questions regarding hospice care and services.
q Participates in community events sponsored by the company.
q Develop and expanding existing book of business throughout assigned area.
q Attend all mandatory job related and annual training meetings.
q Represent the company at community events and business functions.
q Understands and practices the philosophy, culture, and values of Willowbrook Hospice.
q Responsible for submitting a variety of reports, including CRM reporting, as well as growth plans in a timely and accurate manner.
q Maintains the necessary reporting documents, including CRM records, admissions and referral forecasts, pipeline, market trends, competitor activities, and strategic initiatives reports that provide adherence to Compassus policy.
B. Professionalism
q Providing a positive image of Willowbrook Hospice with the general public.
ORIENTATION AND TRAINING
Willowbrook Hospice will provide orientation, training, material, and resources as necessary, to assist Hospice Community Liaison in performing the responsibilities listed above. Training may be in-house, or outside seminars. Evidence of orientation and training will become part of the personnel file.
Work conditions
May require sitting or standing for prolonged periods of time. Driving is required. May require occasional lifting, pushing, or pulling up to 20 lbs.
ACCOUNTABILITY
Reports directly to the Manager of Business Development.
