Some Job Responsibilities:
Provide skilled services to VNSHS patients. Adherence to infection control practices and all agency policies and procedures. Communicate and collaborate with physician to maintain physician orders and report patient assessment and clinical course. Identification of patient centered goals and an interdisciplinary team approach to meet these goals, with the most efficient use of resource and avoidance of re-hospitalization. Develop and revise care plans specific to patient's needs.Maintain knowledge and understanding of Hospice Medicare Benefit. Maintains compliance with narcotic distribution, administration and disposal in accordance with Department of Health and VNSHS policy.Attend care management meetings and discuss entire case load addressing patient's reason for home care and progress towards patient centered goals. Completion of HHA supervisions. RequirementsJob Qualifications:
- New York State license and current registration as a Registered Nurse
- Minimum of one year of experience in acute care setting
- Minimum of three months of home health experience. Consideration of other experience and education preparation will be at the discretion of the Hospice Clinical Manager
- Baccalaureate degree in nursing or related field preferred
