Job Summary:The Hospice Intake Coordinator at University Home Care in Livonia, Michigan is responsible for facilitating the intake process for hospice patients and their families. This position is Full-time and hourly, and the individual will work with the rest of the teams as well as being able to work independently as a contributor to the hospice care team. The Intake Coordinator will play a crucial role in providing compassionate care and support to patients in their end-of-life journey.Compensation & Benefits: This position also offers a fulfilling and rewarding experience by contributing to the well-being of hospice patients and their families.Responsibilities:- Act as the first point of contact for patients and families interested in hospice care- Conduct initial assessments and screenings to determine eligibility for hospice services- Coordinate with healthcare professionals, including physicians and nurses, to gather necessary information for admissions- Collaborate with the interdisciplinary hospice team to develop individualized care plans for patients- Communicate with patients and families regarding available services, expectations, and consent for treatment- Maintain accurate and comprehensive electronic patient records and documentation- Coordinate insurance verifications and obtain necessary authorization for hospice services- Ensure timely and efficient admission processes for patients- Participate in hospice care conferences and meetings as needed- Adhere to all clinical and ethical standards set by the organization and hospice regulationsRequirements:- Minimum of a high school diploma or equivalent; bachelor's degree in healthcare or related field preferred- Previous experience in healthcare, hospice, or customer service at least 2 year preferred- Excellent communication and interpersonal skills- Strong organizational and time management abilities- Proficiency in electronic medical records and Microsoft Office Suite- Ability to work independently and as part of a team- Knowledge of hospice regulations and terminologyEEOC Statement:University Home Care is an equal opportunity employer and does not discriminate based on race, color, religion, sex, sexual orientation, gender identity and expression, national origin, age, genetic information, disability, or veteran status.
