Position Summary: The Hospice Intake RN is an integral part of the intake and admission process and is responsible for managing the eligibility aspect of the referral process for patients entering hospice care, ensuring a compassionate and seamless transition for patients and their loved ones. As such, this person must understand the Medicare Hospice Benefit. The Hospice Intake RN coordinates with multiple other team members via text messaging, phone calls and/or email and conveys pertinent information in a timely manner. Accurate maintenance of the electronic health record is a fundamental aspect of this position.
