If you have a commitment to providing the highest quality of care to patients and their families, we would like to hear from you!Benefits:Pay Range $28.00-$31.00 per hourActual Work/Life BalanceBenefits Package including Medical, Dental, and Vision insurancePaid Time Off401k plan with employer match and 100% vesting after 90 days of employmentA culture with an emphasis on appreciating and valuing the team memberThe opportunity to be part of a rapidly growing national company, with possible position upgradesDetails:Establishes and maintains positive working relationships with current and potential referral sources.Communicate and coordinate patient care with patients, families, referral sources, hospital discharge planners, and clinical staff.Complete phone consultations to educate new patients on the admissions process.Process and coordinate DME orders with contracted local vendors.Track received referrals for reporting purposes.Clerical duties such as copy/file/fax as needed.Observe and enforce company policy and HIPAA compliance.Promote Suncrest culture and mission to ensure quality of care.Qualifications:Minimum of two years' experience preferably in hospice or health care setting, or graduation from a one- or two-year college preferredProficient with Microsoft applications including Word and ExcelStrong time management and organization skillsStrong ability to multi task and prioritizeExcellent telephone and customer service skillsProficient with medical terminologyPNWCWCHP
