Description:The Hospice IPU/Admissions RN is responsible for the transition of patients from hospital care to hospice services, ensuring that they receive comprehensive and compassionate end of life care. Your presence and services form a first impression for patients and families facing terminal illness and should convey comfort and assurance to all involved. This role involves coordinating with healthcare providers, patients and families to provide a seamless transition and support throughout the hospice care process. This position will be Monday through Friday 8am to 5pm working with Emory hospital patients/staff for the Midtown, St. Josephs and Johns Creek locations. Candidate must be a Registered Nurse (RN), licensed to work in the state of Georgia.When covering Emory IPU, care will be performed on site of the appropriate hospital.The IPU Hybrid RN will manage and oversee the care plan and participate in rounds daily of the active patients.Follow up on pending referrals when bed is available.Complete pronouncements of patients at the appropriate time.Assist with home referral evaluations.Attend Emory IDG.When no patients in the IPU/Admissions Hybrid RN will work as a clinical liaison. Responsibilities include:Assess hospice referrals in the hospital setting for patient's need and coordinate discharge.Complete evaluation and admission if needed.Assess and admit patients as needed, driving throughout the Capstone territory. Proficiently documenting in the electronic medical record (Consolo).KEY RESPONSIBILITIES:Clinical Liaison:Manage the transition for patients moving from hospital care to hospice services, ensuring that all aspects of care are addressed.Collaborate with hospital staff to identify patients who may benefit from hospice care and initiate referrals.Coordinate with hospice and hospital teams to develop and implement personalized care plans for patients.Complete evaluation and admission of all qualifying patients.Attend daily rounds with Emory IDG team to discuss patients ongoing needs.Communication and patient/family liaison:Serves as the primary point of contact between the hospital, hospice teams, patients, and their families.- Facilitate clear and compassionate communication regarding patient care plans, goals, and hospice services.- Address any questions or concerns from patients and families about the transition to hospice care.Education and Support:- Offer emotional support to patients and families throughout the hospice care process.- Provide support to hospital and hospice staff on best practices for managing end of life care and transitions.Quality and Compliance:Ensure that the transition process complies with regulatory requirements and hospice care standards.Monitor patient outcomes and satisfaction to identify areas for improvement in the transition process.Observes confidentiality and safeguards all patient related information in compliance with HIPAA regulations.Administrative Duties:Maintain accurate and confidential documentation related to patient transitions and hospice care coordination.Stay informed about updates in hospice care regulation and best practices.Maintain skills and knowledge.Maintain records as required by hospice.Requirements:Candidates must meet the following qualifications to be considered:Education: Nursing degree from an accredited nursing programLicense: Current active Georgia nursing license as a registered nurseCandidates must have the following experience and skills to be successful in the role:Minimum of 3 years of experience in a clinical setting.1-2 years of hospice or palliative care experience and/or IPU experience a plus.Certification in hospice and palliative care, a plus.Proven experience as a hospice registered nurse.Knowledge of nurse care processes and medication guidelines/side effects.Strong nurse assessment skills.Committed to safety precautions and high ethical standards.Current CPR certification.Proficiency in electronic health records (EHR/EMR).Candidates must possess the following traits to be successful in this role:Strong interpersonal and communication skills, with the ability to handle sensitive situations.Emotional/mental stability and stamina.Managing situations that require compassion, empathy and patience.Ability to work collaboratively with healthcare teams and families.Sound judgement and attention to detail.Must be able to manage multiple tasks and priorities effectively.Acceptance of and adaptability to different social, racial, cultural and religious modes.Job Factors:Fast-paced hospital environment with interaction with hospice services and patient families.Must be able to work independently, make judgements based on assessments and plan accordingly.Mental requirements: ability to work independently and make judgements based on assessments and act accordingly.
