What Youll Do:

• Provide supportive care based on patient and family needs in alignment with physician orders and care plans.
• Collaborate with the Nurse Case Manager to deliver personalized care.
• Communicate with physicians, staff, and other agencies to ensure the best use of resources and care coordination.
• Assist with assessments, data collection, and ongoing support for physical, emotional, social, and spiritual factors impacting patient health.

What Were Looking For: Licensure: Current LPN/LVN license in Arizona. Drivers License: Insured and reliable transportation required.?? BLS Certification: Active or willing to obtain. Skills: Strong communication, organization, and emotional resilience to provide excellent patient care.Extras:

• Arizona Class 1 Fingerprint Clearance Card (or ability to obtain).
• Bilingual (English/Spanish) required
• Home Health experience is a plus.

Qualifications:

• Ability to handle emotional stress and respect individual lifestyles.
• Availability for some after-hours and weekend coverage.
