WHAT YOU'LL DO
As an LPN, you will play a vital role in providing comfort and support to hospice patients, ensuring they experience dignity and peace during their final journey. This position requires compassion, commitment, and a deep understanding of the importance of palliative care. You will be responsible for providing bedside care while fostering meaningful connections with both patients and their families.
You will be a valued member of our team, not only as a caregiver but also as an educator to patients and families about hospice care. With a focus on comfort and symptom management, you will work autonomously, making critical decisions at the bedside. Your role will emphasize the importance of hospice care's palliative nature, helping families embrace comfort-focused end-of-life care.
WHERE YOU'LL WORK
Your work will vary daily as you provide care to patients in their homes, assisted living facilities, nursing homes, hospitals, or medical centers. Your schedule will adapt to patient needs, and on average, you will make 4 to 6 visits per shift.
You will also attend regular interdisciplinary team meetings (once a week), collaborating with physicians, social workers, chaplains, and other healthcare professionals to provide holistic care to both patients and their families.
