Apply now to join our team and experience the Transitions difference!As a Transitions Care Regional Director of Clinical Services (RDCS) you will develop, mentor and train leadership within a designated area to ensure quality standards are achieved and maintained and all sites maintain survey ready. The RDCS will establish, implement, and evaluate goals and objectives for hospice services that meet and promote Transitions standards of quality and contribute to the total organization and philosophy. Major Responsibilities
  • Provides guidance and counseling to license level leadership to assist them in continually improving patient care/the patient experience through organization personnel.
  • Assist the assigned locations in setting, tracking and achieving operational & quality goals
  • Assist in the development of/continual reevaluation of effective staffing structures to achieve optimal coverage to ensure quality standards are achieved.
  • Provide guidance and assist in oversight to management of budget items (staffing, medications, nursing supplies, medical equipment, etc).
  • Provide guidance and training to all members of the hospice team on educational materials, educational standards and best practices in patient care.
  • Work collaboratively with the license level leadership on ensuring a state of survey readiness is maintained at all times.
  • Assure compliance with all local, state and federal laws regarding licensure and accreditation standards.
  • Work collaboratively with the license leadership on the learning needs of the hospice staff and other health care professionals, and facilitate formal and informal in-services, and patient care conferences to promote professional growth of all Interdisciplinary team members.
  • Demonstrates behavior appropriate to role model and leader. Adherence to a code of conduct conducive with Transitions HealthCare policy is expected.
  • Demonstrates creativity and critical thinking ability in the development of employees, patient care plans, or projects.
  • Pursues problems to resolution, and /or refers to them through the appropriate chain of command.
  • Interact with all staff in a positive and motivational fashion supporting the Company's mission.

RequirementsEducation and Experience
  • Bachelor's degree in Nursing (BSN) preferred.
  • RN licensed in the state program is located.
  • Five (5) + years general nursing required.
  • Minimum of three (3) years of hospice, home health, medical, surgical, or critical care experience required. Experience with pain and symptom management interventions preferred.
  • Minimum one (3) years supervisory experience in a healthcare setting required.
  • C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred.
  • Ability to work within an interdisciplinary setting.
  • Demonstrate excellent observation, assessment, nursing judgment and communication skills.
  • Strong ability to multitask and prioritize patient needs.
  • Valid driver's license, auto insurance and reliable transportation.
  • Strong working knowledge of all hospice regulations and standards of CHAP accreditation required.
  • Excellent communication skills (phone, text, email)
