You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.Does this describe you? Then you may be a great fit for the Part time/ Per Diem Hospice RN role at AdvisaCare Hospice!!**Current On Call Hours - 5 pm - 8 am Available!- Position requires On-Call responsibilities!** Rotating ScheduleRequirementsInitial and on-going assessment of patient's physical, functional, psychosocial and environmental needsImplement the individualized plan of care and revisions as neededCompliance with AdvisaCare Hospice Policies and Procedures, Medicare complianceConsulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concernSupervision of paraprofessionals providing services to patientPreparing clinical and progress notes, participating in IDG conferencesParticipate in Hospice performance improvement programMaintain high level of customer satisfaction with our clients, families and caregiversCommunication of information using current process and technology within organizationRequirements:Current Michigan License in good standingMinimum 1 year hospice experienceEmpathetic approach to patients and caregiversParticipate in field visits daily, weekly office meetings as indicatedAbility to adapt to patient's status and needs on a full time on call basis
