Join our nursing team at Anthem Hospice! We are seeking a skilled Registered Nurse that will be a Hospice RN Case Manager responsible for planning, directing, and providing in-home care services. This licensed nursing professional will provide direct nursing care and manage a team of therapists and other professionals to treat patients/clients under a physician's plan of care and in accordance with the policies and procedures of the Agency, compliance with the conditions set forth in the State Nurse Practice Act, and regulatory entities. The hospice RN case manager's goals are to provide optimal care and educate the patient and families on hospice services. The provided care plan will include all clinical care, symptom control and identify other physical, psychological, social and spiritual needs for assigned patients.The Company: Anthem Hospice CareThe Hospice RN Case Manager is a member of a multidisciplinary team at Anthem Hospice Care which is a fast growing hospice agency in Phoenix, AZ with a reputation for quality care.We Offer: workable schedules, competitive pay, excellent benefits, training opportunities, and advanced technology such as user-friendly smartphones and software.Key Duties Include:•Follow Agency Policies and Procedures, Federal and State Regulations and Standards of Practice.•Attend Agency In-services, Case Conference, or other meetings as requested.•Provide nursing services in accordance with the orders of a physician and under a plan of care in a patient's home.•Be responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of terminally ill patients.•Records the health needs of the patient.•Performs and completes start-of-care assessment, medication profile, initiates Nursing or Aide plan of care, and associated paperwork in accordance to agency policy.•Evaluates the effects of the care given and regularly reevaluates the patient's nursing needs.•Evaluates and revises the plan of care, as necessary.•Foster a positive relationship with patients/caregivers and doctors to improve patient outcomes.Performance ResponsibilitiesAlthough each position has its own unique duties and responsibilities, the following listing applies to every employee.
