What You Will Do
- Evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.
- Coordinates with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.
- Gathers information and ensures a smooth handoff to the hospice RN admitting the patient/family to services.
- Works closely with the hospital case management department.
- The work schedule is flexible, generally M-F weekdays 8am to 4:30pm .
What You Will Need
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1-2 years recent acute care experience. Hospice or RN Liaison experience a plus but not required. 1 year experience as an RN , preferably in Hospice/Home Care or acute care.
- The RN must be able to communicate effectively, verbally and written
- Valid Driver's License and Reliable vehicle
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
- Understanding of Hospice principles preferred
Training provided in end of life care.

Pay Range: $33.10-$45.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
