Includes the patient and the family in the planning process.Counsels the patient and family in meeting nursing and related needs.Assume responsibility to coordinate patient care for assigned caseload.Completes and submits accurate and relevant clinical notes regarding the patient's condition and caregiving.Attends interdisciplinary meetingsCoordinate interdisciplinary services and obtain physician orders for appropriate careAdminister nursing cares and educationCommunicate continuously with patients, families, and other care team membersRegistered Nurse - RN Case Manager Qualifications:Licensed as a Registered Nurse / RN in the state you work inProficiency with Electronic Medical Records (EMR) software is necessaryPrior Nursing experience in a TCU, Hospice, or Home Health settingValid Driver's License and reliable vehicle with current insuranceExcellent at planning, organizing, and communicatingDemonstrated ability to work autonomouslyAbout Moments HospiceMoments Hospice is a leading hospice organization determined to change the hospice experience for patients, families, and team members.
