Job Description:

Job Title: Registered Nurse (RN) - Hospice Job Specialty: Hospice Care Job Duration: 14 weeks Shift: Monday-Friday, 8 AM - 5 PM, with potential call rotation every 3-4 weeks Guaranteed Hours: 40 hours per week Experience: Minimum of one year of experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases. License: Current license to practice nursing in the state of employment Certifications: Basic Life Support (BLS) from the American Heart Association, current automobile insurance, and valid driver's license Must-Have: - Knowledge of terminally ill patients and their families - Understanding of hospice care - Ability to apply knowledge to the special needs of hospice patients and families - Knowledge of roles of all disciplines providing hospice services - Excellent patient assessment skills - Knowledge of community resources, general nursing practice, managed care principles, and regulatory guidelines Job Description: - Provide direct patient care services to hospice patients and supportive care to families - Manage an interdisciplinary group to enhance the quality of hospice care - Case manage all assigned patient clinical records - Assure the highest quality of care is provided and documented, in accordance with the Plan of Care - Perform initial and ongoing assessment of patients - Develop and implement the Hospice Plan of Care with physicians and interdisciplinary group members - Provide inclusive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support - Report any changes in the patient's condition to physicians - Maintain accurate and updated medical records - Practice infection control measures and standard precautions - Monitor staff scheduling - Participate in Quality Assessment and Performance Improvement Program - Consistently promote core values - Maintain a high level of knowledge in managing pain and symptoms in the dying process - Make routine supervisory visits with team members monthly - Provide patient education and ensure continuity of care with family and significant others - Act as the primary liaison between physician, patient/family, and hospice team - Develop written aide assignments based on patient and family needs - Conduct patient home visits at least every two weeks when aide services are provided, including assessments of aide services
