NDHC's AT Home program, is looking for a Full Time (32 Hours) Hospice Social Worker.
The primary purpose of this position is to plan, organize, develop, and direct all required social services to meet the needs of residents in accordance with federal, state, and local standards, guidelines and regulations, that govern our facility, and as may be directed by the Administrator to assure that the
Medically related emotional and social needs of the resident are met/maintained on an individual basis.

Strong background in mental health/counseling and experience in health services delivery
Ability to work with other members of the Interdisciplinary Team
Ability to conduct assessment of patient/family psychosocial history
Ability to identify and prioritize problems with appropriate goal formulation and conduct required interventions
Demonstrated leadership capabilities with the ability to relate well to staff and other professionals
Valid state driver's license and reliable automobile with proof of current automobile insurance
Expected to participate in appropriate continuing education as required to maintain licensure
Must possess excellent verbal and written communication skills.
Typical workday hours are Monday through Friday.
