Specific attention to meeting the immediate care needs as related to funeral and advance directives.* Responds to referrals, conducts hospice informationals and leads the consent process for new patients.* Completes all charting and visits timely in accordance with state and federal regulations, as well as agency policy and procedures.* Participates in the care planning and case conference process ensuring the involvement with patient/family and that their needs are accurately assessed and care planned.* Provide external referral to community supports to patients, families and community facilities as requested.What do I need to be considered?* STAR mentality (Selfless, Tough, Accountable, Respectful).* Bachelor's degree from accredited Social Work program required.* At least two years of experience in Social Work, Case Management or related field.* At least one year of social work experience in a health care setting required.* At least one year of experience in Social Work in hospice/home care preferred.About Hillcrest:As a local organization, we offer a tight-knit, friendly environment with the benefits and options of a large company!
