JOB TITLE: Team Director
JOB SUMMARY: Coordinates all aspects of patient care. Serves as the team leader of their prospective service area, providing direction and supervision to the Hospice Interdisciplinary Care Team, (direct care staff and contracted staff). Serves on the Provider Relations Team. Serves as a consultant to hospice staff. Supports the Provider Relations and Quality Improvement activities of the organization. Ensures compliance with all State and Federal Hospice and Nursing regulations.
RESPONSIBILITIES:
1. Supervises all direct patient/family care staff, including Nurses, Social Workers, Home Health Aides and Spiritual Care Coordinators.
2. Supervises coordination with Medical Director and attending physicians during the initial Plan of Care development at admission and as the Plan of Care is updated.
3. Ensures that all patient/family services comply with Federal, State and local regulatory requirements in accordance with professional standards.
4. Participates in the development, implementation and evaluation of policies and procedures for patient/family services.
5. Actively participates in the selection and evaluation of all direct patient/family care staff.
6. Coordinates the development, implementation and review of the Interdisciplinary Plan of Care for each patient/family.
7. Maintains the Hospice patient computer generated clinical records for each service provided, to each patient, according to established regulatory standards and agency policy.
8. Conducts the interdisciplinary team meetings and maintains appropriate documentation, attendance and all changes within the patient Plan of Care.
9. Ensures the 24-hour availability of nursing services to patients/families creates and coordinates the on-call Schedule.
10. Ensures that the initial patient assessment of a patient who requires the services of a Registered Nurse, licensed Nurse, Hospice Aide/Home Health Aide, or other Nursing support staff is performed by a Registered Nurse.
11. Participates in Quality Improvement reviews, especially related to the evaluation of Nursing services and IV therapy services, and Provider Relation activities. Makes recommendations for the overall improvement and enhancement of agency standards
12. Communicates as necessary to facilitate optimal patient/family care with all members of the IDT including: Hospice Administrative staff, Medical Staff, Nursing Staff, Volunteer, Spiritual and Bereavement care staff, counselors, therapists and facility staff when applicable.
13. Participates in the orientation and ongoing education of staff and volunteers.
14. Participates on committees and special projects and other duties as assigned.
15. Initiates preventive and rehabilitative nursing procedures.
16. Reports changes in the patient's medical and mental condition to the attending physician as well as to other hospice staff and contract employees involved in the patient's care.
17. Manages the development of a discharge plan when a patient is discharged prior to death or transferred to another hospice.
18. Participates in quality improvement program and the evaluation of nursing and other clinical services.
QUALIFICATIONS:
1. Registered Nurse currently licensed in the state pursuant to Article 139 of the New York State Education Law.
2. A Baccalaureate degree in nursing and four years experience in delivering nursing services with at least two years experience in a supervisory or administrative position; or 6 years of experience in delivering nursing services with at least 2 years experience in a supervisory or administrative position.
3. Knowledge of Computer operation, computer generated clinical records experience preferred.
4. Unless ALL assigned patients can be served by using public transportation, must have reliable automobile, valid driver's license, and the minimum state required liability automobile insurance.
