Performs Patient assessments: Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals. Develops the plan of care for the patient and family

Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement

Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours. Completes re-visit documentation and transfers

Homecare/Hospice RN: Hospice and experience Days per week: 5 Shift: 8a-4p On call or weekends required? 1 weekend per month and possibly an additional day on a weekend Float: Yes, from county to county (We cover Morris, Somerset, Union, and Essex) Our greatest need is in Morris County Call Back: No

Must have 2-3 years as RN in home hospice environment

Must have valid New Jersey driver's license

BLS - required

Job Qualifications/Experience:

Current New Jersey RN License

Experience using an EMR, excellent critical thinking and communication skills
