- Be the First Line of Support: Triage patient and caregiver calls, expertly assessing their needs and guiding them toward solutions ensuring adequate symptom management.
- Support the Clinical Field Team: Provide critical support to field staff with medication refills or adjustments, and Durable Medical Equipment (DME) orders and delivery.
- See the Bigger Picture: Gather and document key physical, psychological, social, and spiritual insights to ensure holistic care, initiating interventions with the interdisciplinary team when needed.
- Be a Steady Presence: Step in with in-person support for urgent patient needs, offering comfort and skilled care when it matters most.
Requirements:
- A Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license.
- Experience in hospice or palliative care (2 years of experience preferred).
- A skilled communicator with strong clinical assessment and critical thinking abilities.
- A team player who brings both compassion and professionalism to every interaction.
