* Collaborate with the physician to oversee and implement personalized care plans, ensuring the active involvement of interdisciplinary teams to achieve the optimal outcomes for each patient

* Perform initial and comprehensive assessments on all patients in accordance with policies and procedures
* Provides high-quality patient care while ensuring clear, concise, and accurate clinical documentation
* Assesses the need for plan of care updates while keeping the patient and family involved in care planning decisions

What it takes to become a Hospice Visit Nurse:
* RN with a minimum of two years of recent clinical nursing experience.
* Current, valid CPR certification.
* Demonstrated patience and a supportive and sensitive approach to providing patient, family, and caregiver support.
