Responsible for providing internal medicine services to patients. Primary duties may include, but are not limited to: Creates a comprehensive and coordinated plan of care across all settings including hospitals, clinics, and Skilled Nursing Facilities (SNFs), which take into account medical records, patient and family, primary care physicians, and specialists. Makes hospital rounds daily. Collaborates with hospitals and care management teams and reviews patients with case managers. Attends to patients in our care center and assists nurse practitioners in reviewing cases. Attends to patients in the SNFs and attends SNF meetings to review cases. On call nights and weekends as scheduled.
The Hospitalist provides Internal Medicine Services to patients.
Essential Duties and Responsibilities
1.Round in the hospital Monday - Friday.
a. Work with the Hospital, Care Management Team and other Hospitalists to ensure all patients are seen daily.
b. Conference call with Case Managers to review patients, and discuss the discharge needs and plans.
c. Admit the patients from the ER patients in the afternoon (usually 2 to 4 patients), if they are assigned ‘float' position for the given day.
d. Work with Case Managers in transferring the patients from ‘out of area' hospitals into network hospitals.
2. Hospitalist in the clinics
a. See patients in the care center All patients discharged from the hospital are seen by the hospitalists in the clinic until they are stabilized.
b. Patients with falls are assessed.
c. Pre operative clearance is done on patients undergoing surgeries requiring general and spinal anesthesia.
d. Assist Nurse Practitioners by reviewing the cases with them.
3. Hospitalist in the Skilled Nursing Facilities (SNFs)
a. See the ‘skilled' patients in the SNFs. These patients are seen once a week until they remain skilled, which is normally from 1 to 2 weeks.
b. Attend the SNF meetings once a week to review the cases.
4. Across all settings of care, create a comprehensive and coordinated plan of care which takes into account information from medical records, the patient and their families, primary care physicians, and specialists as necessary.
a. Medical records to be completed in a timely fashion:
History & Physicals completed within 24 hours of inpatient admissions.
Discharges plans coordinated with the Care Management Team on the day of discharge. Clinic visit notes on the day of the visit
Communicate with patients or their families/care givers and the Primary Care Physician during the admission about the plan of care and any changes in the patient's health condition.
Work with the Care Management Team to ensure the member is supported when care is transitioned from one setting to the next.

1. Completed Internal Medicine or Family Medicine Residency in the United States
2. Completed medical school
3. DEA
4. Must be board certified or board eligible in specialty
5. Must actively be doing inpatient work
