Benefits are noted, such as decreased length of stays, increased face time with providers, increased transition care focus, and patient education. Goals would also be to enhance throughput efficiency. Creating a space for more direct provider care and less indirect care activities are paramount. Increasing provider satisfaction is also noted in facilities that have published their experiences. Much of the research is anecdotal at present, and the role is flexible based on facility need.This person acts as a liaison from patient to provider to case management for needs post hospital to be better provided for.
