New Horizons is hiring a housing-focused Case Manager to engage with youth and young adults experiencing homelessness up to age 25, across the spectrum of religious beliefs, race, ethnicity, culture, sexual orientation, and gender. The Case Manager will join a direct service team and coach youth to develop their full potential, with a strong emphasis on housing solutions. This requires in-person work within group and one-on-one settings. The schedule is structured to be available to youth in shelter, engagement programs, and in the community. This is achieved with two anchor shifts (approximately 4-8 hours per week) in milieu programming onsite. Each Case Manager's scheduled shifts (may occur in the morning, evening, and/or weekends) are determined through a collaborative process with the Housing Solutions Team based on need and availability. Outside of weekly scheduled shifts, Case Managers have broad flexibility to meet youth onsite, during programming, and/or in the community. The Case Manager effects change in the lives of youth and young adults (YYA) by providing a full range of case management services, including, but not limited to, engagement, assessment, intervention, short-term counseling, goal setting, and referrals for a caseload of 20-30 young people.
