The Housing Case Manager works as a member of a multi-disciplinary behavioral health care team providing integrated behavioral health services to clients seen at Old Town Recovery Center (OTRC). The Housing Case Manager is responsible for providing housing support the OTRC mental health teams, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered.The programs at OTRC address all treatment needs, including crisis management, substance abuse treatment, psychiatric evaluation and treatment, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing. The programs are responsible for proactively managing the healthcare needs of the clients on their assigned team, including medical, mental health, addiction, and health behavior needs.Schedule: Monday-Friday 8:00am-4:30pmLocation: Old Town Recovery CenterWage: starts at $23.69/hrSeniority Bid Window Closes: 9/25/2024Close Date: Until filledMINIMUM QUALIFICATIONS:Bachelor's degree in Behavioral Sciences field or a combination of at least three year's work, education, training or experience in substance use disorders and/or mental health treatment;Has the competencies necessary to:Communicate effectively;Understand addictions and mental health treatment and service terminology and to apply the concepts;Provide psychosocial skills development; andImplement interventions prescribed on a treatment plan.Must be registered with the appropriate Oregon state licensing board to meet, at minimum, QMHA requirements.Experience with severe and persistent mental illness (SPMI) preferred.Bi-lingual (English/Spanish) preferred.Must meet CCC privileging requirements as required by FTCA.Must have current CPR certification prior to start date.Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.If using a vehicle for agency use, must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access.
