DescriptionJOB TITLE Housing Facilitator SUMMARY: Establish, promote and maintain the Creating Housing Initiative Grant through TN Department of Mental Health.EDUCATION AND EXPERIENCE:Education: Bachelor's degree required; Master's degree preferred.Licensure: Must possess the highest level licensure appropriate to degreeValid driver's licenseCertification: N/AExperience: Experience in clinical practice, case management or housing preferred.Knowledge/Skills: Experience with disability issues and housing.Skilled in use of all major computer applications.Verbal/written communication skills.EQUIPMENT:Computer, fax, copier and any other equipment required to perform the functions of the position.MAJOR DUTIES AND RESPONSIBILITIES:1. Follow Scope of Services outlined in Regional Housing Facilitator, Tennessee Department of Mental Health Grant. Critical items include but are not limited to:a. Establish and assist in maintaining local Housing Initiative Steering Committee for Region Ib. Conduct local permanent housing assessmentsc. Create and execute a local strategy expanding permanent housing and supportive services optionsd. Develop and maintain a local housing resource mechanism2. Maintain current records and information required to TDMH.3. Keep supervisor apprised of all critical or emergency situations.4. Maintain current professional license infield of expertise.5. File all reports in a timely manner.6. Attend and participate in regularly scheduled staff meetings and inservices and individualprogram planning staffings as needed.7.
