Together we can make a differenceEliot's Homeless Services Division has received funding to open a low threshold housing site in Lynn, MA. We will house individuals experiencing street homelessness and will include an array of onsite supportive services, including recovery coaching, MAT/Psychiatry, wrap-around case management & clinical coordination, medical consultation and direct interventions, and access to trauma-focused treatment modalities. Eliot is seeking a creative/passionate Housing First Coordinators to provide onsite case management & support in a highly collaborative fashion with residents. Successful candidates will have unwavering compassion, as well as an open mind in terms of pathways to recovery/harm reduction with respect to substance use disorders. Working knowledge of area resources/providers, housing navigation, and tenancy preservation are a plus. Candidates with lived experience as strongly encouraged to apply.Responsibilities:
- Empower residents with exercising autonomy and independence in all aspects of their life. Assist with identifying choices and developing skills in self-advocacy.
- Collaborate across the team at all levels to ensure that resident perspective is always included, that preferences are recognized and that choices are maximized.
- Provide guidance to clients in recovery or harm reduction process. Provide advocacy, support, and education during orientation and throughout service delivery.
- Assist clients in being fully integrated into their communities by supporting them with resource utilization and/or obtaining memberships of interest (e.g. gym, library, civic associations).
- Provide practical help and mentoring/modeling, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain skills needed for daily independent living.
- Promote concepts of health and wellness by assisting clients with accessing information and services related to medical care including HIV care resources, nutrition, and exercise as appropriate.
- Work within the multidisciplinary team to guide residents through legal concerns.
- Intervene in potential crisis situations, utilizing trauma informed, verbal de-escalation techniques - escalate concerns to leadership/on call personnel in real time as applicable.
- Educate and support clients around harm reduction principles and utilization, including access to safe injection supply kits when requested.
- Assist clients in time management and attending appointments as independently as possible (e.g. managing anxiety when utilizing public transportation). Provide transportation and appointment support as assigned. Coordinate with collateral providers as needed.
- Actively participate in house community meetings to obtain feedback from residents and plan educational and recreational activities with people served.
Qualifications:
- Bachelor's degree in human services or related field required.For candidates with lived experience with substance use disorder who have 2 years sustained sobriety.
- High school diploma/GED and at least one year of paid or volunteer experience working with adults with mental illness and/or substance abuse.
- Must have a strong commitment to substance use recovery and housing first philosophy.
- Must have skills to establish supportive relationships with persons with mental illnesses/substance use and respect for their preferences for treatment.
- Ability to provide flexible services that are community based and focused on team work.
Schedule: Sun and Mon: 7 am-3 pm Tues, Wed, and Thur: 1 pm-9 pm
- Full time, 40 hour per week position
Salary: $48,000 Bachelors and $50,000 Masters level6% salary increase for a bilingual candidate in needed language areas.
