Job Description:
Eliot is in search of a Bachelor's level Housing First Coordinator Supervisor with at least 1 year of experience in a case management supervisory capacity in the human services realm. Our ideal candidate will have unwavering compassion and empathy - along with a strong commitment to social justice, person centered planning and practice, and multidisciplinary collaboration. Eliot's Permanent Supportive Housing Teams provide multidisciplinary trauma informed/strengths based services to effectively address barriers to obtaining and maintaining housing (eg untreated PTSD/Mood Disorder(s)/Psychosis/Substance Use Disorder(s)/medical complexity)Responsibilities:
  • Conducts and supports assigned supervisees with screening and assessing family needswithin initial intake. Flag high risk factors reported or observed and immediately report toSite to formulate a plan of action to support the family.
  • Ensures appropriate milieu coverage in coordination with Site Director as needed to ensureoverall safety of the site and to ensure that community rules are followed.
  • Provides dedicated 1:1 supervision with assigned Housing First Coordinators weekly
  • Assists guests with transportation arrangements and tangible resources to address immediateneeds in conjunction with Housing First Coordinators and other shelter personnel
  • Carries an assigned caseload of families and collaboratively coordinates care and supports toaddress assessed needs and service delivery preferences
  • Ensures that supports provided to families are culturally and linguistically appropriate, familycentered, and trauma informed
  • Provides onboarding/onsite orientation to new personnel as assigned. Participates ininterview/hiring process for new staff as needed/assigned.
  • Provide comprehensive service and care coordination through collateral contacts as indicatedwhile leveraging pre and post tenancy support services within a Housing First framework.
  • Complete assessments for HomeBase, Coordinated Entry and/or DMH Services
  • Assist with and participate in strategic planning for the Family Shelter Site.
  • Directly support families (and assigned supervisees) with maximizing state and federalentitlements
  • Collaborates closely with housing navigation team
  • Recognize and respond to signs and symptoms of changes in client behavior, physicalhealth or related medical issues, or other related areas of concern
  • Obtain and maintain current certifications in CPR, First Aid, and all other required training.Ensure that all certificates are current and provide required documentation to the Agency.
  • Develop and modify individual client service plans based on changes in needs
  • Other related duties as assigned
Qualifications:
  • Bachelor's degree in social work, counseling, psychology, or other closely related field
  • Ability to speak Haitian Creole, Spanish, Portuguese or French
  • At least 1 year of experience in a case management supervisory capacity.
  • At least 2-years experience with the homeless population
  • Must have demonstrated experience with family centered engagement, support planning/coordination and service delivery implementation.
  • Knowledge of Public Entitlements (EAEDC, Social Security, Immigration, etc.,) and other resources is required.
  • Has working knowledge related to EA Shelter system, housing navigation and tenancy sustaining supports within a Housing First framework.
