Job Description:
Housing First in Appalachia (Part-Time)ARCH will follow Housing First evidence based practices (EBP) to provide a coordinated system that allows for equal access to housing services with minimal barriers to entry by establishing Access Sites throughout the CoC. This Coordinated Entry (CE) process is linked to street outreach efforts so that people sleeping on the streets are prioritized for assistance in the same manner as any other person assessed through the CE process. The CE process must ensure client immediate needs, such as housing, are assessed and addressed quickly first (Housing First), and continue to be regularly assessed for ongoing service needs through case management. ARCH has identified the need for 1) the coordination of services for development of a Crisis Response System to ensure low barrier entry to rapid rehousing and permanent supportive housing with wraparound mental and behavioral health services for homeless and chronically homeless individuals, Veterans, and homeless unaccompanied youth with mental illness, co-occurring disorders and substance use disorders; 2) support systems necessary for housing stability; and 3) the development of affordable, safe, healthy housing. Further help on this page can be found by clicking here.Member Duties : AmeriCorps members will perform outreach activities to identify, engage and inform the homeless of CE Access Sites. AmeriCorps members will perform screening intake to determine eligibility and Vulnerability Index Service Prioritization Decision Tool (VI SPDAT) acuity assessments, and initial SSI/SSDI, Outreach, Access and Recovery (SOAR) interviews. AmeriCorps members will assist participants receive housing services find and access appropriate housing; find resources to support appropriate housing; complete housing application; connect with mainstream benefits; connect with income supports, primary and mental health services and substance abuse treatment; and assist with home management skills and employment support. AmeriCorps members will help precariously housed clients obtain housing supports to prevent homelessness.
