LifeSTEPS is hiring a full-time Housing Navigator/Intensive Case Manager for the Alameda County, CA area! It is also essential to provide meaningful programs that empower people to move forward in their lives.About the PositionThe Housing Navigator/Intensive Case Manager position assists and aids individuals and families requiring assistance from the organization. The Housing Navigator/Intensive Case Manager fills a central role in linking homeless individuals and families to an appropriate housing plan and supporting them in Tenancy Sustaining Services. The goal for the Housing Navigator/Intensive Case Manager is to facilitate rapid and resourced exits from homelessness into permanent supportive housing and provides participant advocacy, case management, benefit establishment, linkage to Mental Health and/or Substance Use Services, linkage to stable housing and all other supportive services as needed.Core Duties (but not limited to) Housing Navigator / Intensive Case Manager:Assess participants to identify needs such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of assistance.Promote, coordinate and tailor services to meet identified needs.Must be physically at each assigned property as scheduled and spend the required contracted hours at each assigned property per month.Assist participants individually, with family, or in other small groups regarding plans for meeting needs; aid participant to mobilize inner capacities and environmental resources to improve social and economic functioning.Facilitate the housing planning process, including oversight of barriers and putting eligible participants on a path to maintaining housing.Coordinate appropriate systems to place guests in permanent housing and set up any necessary supports, such as utilities and furniture.Remain primary point of contact between the guest and landlord up until and after the signing of the lease or sublease.Remain the agency's primary point of contact for landlord relationshipsCoordinate options for moving/maintenance issuesAddress lease-related issues on all supportive housing unitsOversee the development and implementation of individual success plans in the housing stabilization program, including identification of barriers to obtaining/maintaining housing and steps to overcome them.Advocate and refer participants to community resources, human services and other organizations as needed.Identify need for and assist in development of auxiliary services to facilitate bringing applicants into job ready status.Provide employment support (skills assessment, job applications, cover letters and resumes, job search, and interviewing techniques.Review service plan and perform follow-ups to determine quantity and quality of services provided participants and status of participant cases.Compensation & BenefitsThis is a full-time, non-exempt 40 hours per week position, with a starting pay rate at $28.40 per hour. We provide an attractive benefits package including two weeks paid time off to start, paid holidays, medical, dental, vision, life insurance, a 401(k) plan, and an employee assistance program.QualificationsEducation and/or Experience and/or License Certification:Bachelor's degree in social work, psychology or related field.Two (2) years of experience in a similar environment providing social services and housing navigation.Bilingual English/Spanish preferred.Highly motivated self-starter and ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment.Computer skills with proficiency in Microsoft Office.
