Job PurposeThe overall purpose of the Housing Navigator/Case Manager is to ensure that all housing procured for the Supportive Services for Veteran Families (SSVF) program meets all the standards as provided by the Department of Veteran's Affairs (VA) and the SSVF program. The incumbent will also provide comprehensive case management, financial counseling, and assistance to assist veteran families to become stably housed.
Qualifications- Bachelor's degree in social services and or a combination of education and experience of outreach and housing
- Valid driver's license, current insurance, and reliable transportation is required
- Minimum two years of experience in case management, life skills as well as conducting assessment and counseling services
- Experience working with Veterans is preferred
- Experience delivering services to SUD, COD, or homeless populations is also preferred
Essential DutiesHousing NavigatorDevelop and implement an individual housing plan for each veteran familyAssist Veterans with completing housing applications and securing housing of their choice Educate veterans on landlord & tenant rights and responsibilities and how to be a good neighbor Ensure that Veterans have the necessary items to secure housing i.e. income verification, I.D., etc.Act as a liaison between SSVF, VA (HUD-VASH), and Veteran families.Advocate to help address issues and barriers between Veteran and landlord that may prevent the participant to move in.Assist Veterans with obtaining housing by providing housing leads via; the internet, housing authority, landlord database, etc.Develop relationships with property management, property owners, and low-income housing programs. Conduct weekly follow-ups to ensure progress is being made towards the veteran's housing goal.Provide and support linkages to community organizations to obtain furniture/household items etc. Maintain communication with the HUD/VASH social worker and veteran.Maintain up-to-date, accurate, and complete veteran interactions and services records by writing case notes in HMIS. Request temporary financial assistance after obtaining approval from the Supervisor.Actively participate in case consultation, staff meetings, training, etc. Transport veterans/attend to perspective housing unit appointments.
