Job Type Part-timeDescriptionThis is a Part Time Position• Under the supervision of the Regional Service Manager, performs duties relating to housing placement Housing Support Specialist will work with clients in need of short-term housing or long-term housing and identify stable living for them Work closely with the Housing Placement Specialist, providing support to PWH towards permanent stable living Tasks/Duties/Responsibilities70%Client Services • Screen clients referred for (Housing Placement Assistance) HPA and identify individuals in need of housing assistance, using agency intake and assessment forms;• Identify permanent housing for clients. • Inspect apartments utilizing Hyacinth standard checklist; • Assist clients with placement in transitional housing. • Negotiate with landlords and provide complete assistance with lease and other documents.• Documentation of baseline viral load, CD4 count, AIDS diagnosis and antiretroviral prescriptions for 100% of clients. Conduct counseling sessions with clients pertaining to housing needs, options, and social service referrals.Work closely with landlords and utility companies to ensure that service will not be interrupted.Work with the Housing Ambassador of Middlesex County for the State's Housing Collaborative.Develop and implement outreach activities including networking with other community- based organizations, support groups, religious organizations, etc. Participate in outreach activities for high-risk groups.Develop a database of work15%Reporting/Monitoring Establish, maintain and update client case records using approved agency forms and procedures. Ensure confidentiality of all records.Prepare monthly reports in accordance with agency policies and procedures.Coordinate availability of funding with Finance Department and Client Services ManagerRefer and advocate for client placement in permanent and transitional housing programsMaintain and update client information in eCOMPAS and CAREWare10%Hyacinth representation/advocacy Attend and participate in staff meetings and supervisory conferencesMaintain working relationships with other community providersObserve established administrative procedures.Provide written reports and maintain statistics as requiredRepresent the agency at community events, meeting and conferencesAttend and participate in staff training opportunities.Adhere to Hyacinth policies and procedures including client confidentiality provisions5%Training and certifications • Attend Middlesex County TGA Planning Council• Participates in Middlesex COC monthly meetings 100% TotalRequirementsRequired Minimum Education: Associate's DegreeRequired Years of Related Experience: 1-3 years• Social services/community services
