These services may include providing general information about various housing programs, crisis intervention services, developing action plans, providing an appropriate level of guidance and support, facilitating referrals, and serving as an advocate on behalf of participants to assist them in securing services and support to reach their housing goals.Responsibilities:Assess displaced client and family needs out in the fieldMeet with clients at their home or where they are currently residingDevelop comprehensive housing plans for families to become self-sufficientCoordinate needed servicesDevelop links with a continuum of services and agendasMaintain up-to-date client recordsProvide crisis and/or short-term housing counselingProvide information and referral to community and staffEngage in outreach activitiesEnsure compliance with all Department of Housing and Urban Development policiesProvide initial and ongoing client assessment; development, implementation, and evaluation of a housing plan; coordination, and monitoring of supportive services in accordance with the client's needsProvide general information about housing, screening of applicants for acceptance into various housing programs as well as verifying eligibility for such opportunitiesActively recruit landlords willing to work in conjunction with the client and locate affordable housing opportunities to meet the needs of the client populationThis position requires some evenings/weekendsSkills/Abilities That Are a Must-Have:This position spends 80% - 90% of the time in the field working in the Moreno Valley, CA areaMinimum of 2 years' case management experience; knowledge of homelessness, addiction and mental health issues as they pertain to provisions of supportive serviceDemonstrated communication skills and sensitivity to various cultures and socio-economic groupsWorking knowledge of computer skills, including proficiency with Microsoft PowerPoint, Word and ExcelBilingual (Spanish) preferredCurrent California Driver's License, clean driving record, proof of insurance, reliable personal vehicle, and a willingness to travel offsiteKnowledge of and experience with motivational interviewing preferredMust have a valid CA driver's license, insurance, a reliable personal vehicle, and a willingness to travel off-siteAbility to pass a pre-employment drug testSkills/Abilities We'd Like You to Have:Baccalaureate level of conceptual thinking, organization and expression obtained by a degree in Sociology/Psychology/or other related field or equivalent work experienceAdditional Information Required Upon Hire: Physical Demands of the Job (reasonable accommodations may be made, as appropriate)Must be able to:Sit, talk and listenStand, walk and use hands and arms to use a keyboard, handle papers, and reach for the phoneOccasionally climb or balance, stoop, kneel, crouch or crawlOccasionally lift and/or move up to 20 poundsConcentration and good hearing are requiredImportant Notice:Incomplete submissions will not be considered.
