Position Snapshot: This hourly position reports to AIMs Director of Housing and primarily assists persons with disabilities and the community by conducting the intake process for housing, information, and referral, monitoring tenant's housing needs, coordinating activities for tenants as well as IL advocacy, support, and community change including the areas of housing.

Candidate Requirements need to include:

+ Bachelors Degree in social work, human services, or other social science; plus

+ Two (2) years of experience as a service coordinator or case manager in an appropriate facility.

OR

+ Associates Degree in Human Services or Social Work with 5 years of applicable experience.

Candidate Knowledge/Skills/Abilities:

+ Unwavering commitment to quality programs, reliable work ethic, and independent supervision.

+ Proven record of strong business acumen skills that provide a high-level job performance to fulfill the agency's mission.

+ Experience advocating and facilitating IL services for persons with disabilities and seniors.

+ Ability to work effectively in collaboration with diverse groups of people.

+ Passion, idealism, integrity, positive attitude, mission-driven, and self-directed.

+ Knowledge of the Tri-County community resources to support information and referrals.

Essential Functions:

The ISH HSC performs a wide range of duties, including but not limited to:

+ Intake and Advocacy: Assist and support applicants in applying for Independent Living Services.

+ Organization: Ensure program activities operate within the policies and procedures of AIM and OPWDD.

+ Information and Referral: Provides community outreach to individuals and/or groups regarding housing.
