We follow a Housing First model and philosophy and work to keep families - all along the continuum of homelessness - safe with a roof over their heads. As a Housing Stability Case Manager, you will give families the support they need at the most pivotal times, building on families' strengths, honoring their choices, and taking a whole-family, multi-generational approach. This is a site-based position responsible for providing client-centered case management services involving home visits, referrals to community resources, goal planning, and community collaboration. You will have a direct impact in people's lives.The pay rate for this position ranges between $25.00 - $25.96 per hour and the offer amount is determined by the candidate's education, qualifications, and experience . Highlighted Duties and Responsibilities:Support families in gaining and maintaining housing stability through developing case management plans that incorporate goals for both adults and children related to core areas: finding stable housing; increasing income; and family stabilization and support;Meet face to face with families on caseload in their homes to pursue goal-related tasks, review progress, and address barriers to accessing services;Conducts scheduled and unscheduled home visits and provides tenant education in accordance with program contract guidelines, and as needed.Works consistently within a housing-first framework (i.e., providing clients with immediate access to available independent permanent housing and supportive services without prerequisites for sobriety or participation in psychiatric treatment), ensuring that services offered are strengths-based and client-centered.;Network with community resources for housing, employment, emotional wellness services, training, etc., and connect families as appropriate, including to COH health services;Engage families and assess needs, preferences, and strengths;Demonstrate empathy and flexibility in responding to families' needs and requests while fostering independence;Act as a liaison with landlords and other community supports as needed;Participate in multidisciplinary meetings, trainings, and regular supervision.
