HRDI is seeking a Case Manager to join their team. The Case Manager will be responsible for assessing appropriate levels of care based upon comprehensive criteria and identification of appropriate services both within HRDI and externally. This role offers a competitive salary and benefits package.Job Duties:Must be available, on a rotational basis for 24 hours, to intervene during crisis situations.Works closely with State Mental Health Facility to plan for services needed or high rates users referredbecause they are to be discharged from inpatient care.Works closely with community support treatment team triage.Maintains professional working relationship with all referral agencies.Develops plans to refer, link, and place individual with community based treatment and other neededresources.Assists individuals by referring/linking, placing them with recreation, transportation, financial andmedical entitlement, vocational rehabilitation services, as appropriate as needed.Conducts outreach and case findings.Determines eligibility and conducts comprehensive assessment of service needs.Develops the comprehensive master Individual Treatment appropriate housing facilities, with the input ofthe client and/or family unit.Reviews Individual Treatment Plan of clients regularly to assess effectiveness and need for specific planelements, with input of the client customer and/or family.Responsible for locating appropriate housing facilities as needed.Provides supportive counseling to individual client and/or family as needed.Provides problem solving assistance to client and/or family as neededProvides public education on services available and needs of the mentally ill as opportunities ariseand/or as assigned by the Vice President.Documents all services provided in progress notes and must perform DIS billings of service on timelyJob Requirements:Bachelor's Degree in related human services/social services fieldValid Driver's License and vehicle required to travelTwo-three years experience in mental health setting preferredKnowledge of utilizing DMS IV and ASAM criteria for purposes of assessing clinical diagnoses andappropriate client placementKnowledge of medical needs and available resources to the psychiatrically disabled persons.Knowledge of appropriate record keeping proceduresAbility to work as part of a multi-disciplinary team to ensure continuous improvement in all serviceareas.Relates well to people of all socioeconomic groupsKnowledge of computers and applicable softwareExcellent Writing SkillsKnowledge of languages other than English helpful, particularly Spanish
