We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing. # General Responsibilities: Responsible for helping residents obtain supportive services from the community that are needed to enable independent living and aging in place.# Refers and links the residents to supportive services provided by the Community such as, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy. # Provides case management when service providers are not available through the general community. Creates a directory of providers for use by both development staff and residents. # Education/Training: A Bachelor#s degree in Social Work, Gerontology, Psychology, Counseling or related field of study normally required. # Experience: A minimum of three (3) years# related experience normally required. # ADA: Ability to ability to bend, stoop, sit, reach, and lift items weighing up to 25 lbs.; visual and auditory skills. # Compensation and Benefits: Diakon offers a competitive compensation and benefits package for Full-Time and Part-Time employees.
